Please have a copy of the district’s State Allocation Board (SAB) approved funding item, a copy of the completed Fund Release Request (Form SAB 50-05) or Expenditure Report (Form SAB 50-06) that is being submitted with the PIW, and a copy of the completed Application for Funding (Form SAB 50-04) that was submitted to the OPSC for this project. Some of the requested information can be attained from these documents. Please contact the OPSC if you do not have a copy of the funding item and/or Form SAB 50-04.

The “Project Information Worksheet” currently only offers limited browser support. If you experience any issues, we recommend using Internet Explorer.

Important Note: The submittal process has been changed. As of (October 29, 2009), users will enter the OPSC Application Number to create a new PIW instead of the Project Tracking Number (PTN). Additionally, multiple OPSC Application Numbers can now be entered on one PIW in the event that the projects are completed under one contract. Instructions for completing a PIW with multiple OPSC Application Numbers
a.	To create the PIW, enter the OPSC Application Number for which you are submitting the corresponding SAB Form 50-05 or SAB Form 50-06. This will be the “Primary” application number. Once the PIW has been created, enter any additional application numbers in the “Secondary Application Number” field in the District Information section of the PIW.
b.	Provide the combined funding, costs, and project information for all application numbers listed in the OPSC Application Number field in the District Information section. Do not provide information that is part of an application number not listed in the OPSC Application Number field (i.e part of a modernization project).
c.	PIWs submitted with multiple OPSC Application Numbers will only meet the PIW submittal requirements for the project listed as the Primary OPSC Application Number. Additional PIWs will need to be submitted for any projects listed as Secondary OPSC Application Numbers and should include the same, combined information of all other application numbers listed on the previous PIW.
d.	Districts must provide an explanation in the Comments filed in the Additional Information section at the end of the PIW as to why they completed the PIW using multiple application numbers.
